To begin the process of requesting data from the Kentucky Department of Education (KDE), please know the following:
  • All requests begin with this form.
  • The KDE Data Policy Committee is notified and given opportunity to review all requests.
  • Additional information may be required to fulfill approved requests.
  • A Memorandum of Understanding will be required for disclosure of personally identifiable (PII) student data.
