The KDE Online Data Request Form must be submitted for all internal and external data requests. External data requests must:

  • Be allowable under applicable law;
  • Align to KDE’s Strategic Plan; and
  • Include sponsorship by a KDE associate commissioner and be approved by the KDE Data Governance Committee.

Upon notification that an external data request has been approved, a memorandum of understanding (MOU) may be required. KDE MOU templates and information are provided on the KDE Data Request webpage.

If requesting personally identifiable information (PII), select Student option. For aggregate level, select School, District or State.




Attachments

You may attach up to three files to provide additional information. The system will only accept Word, Excel and PDF files up to 4 MB in size.

Do not attach files containing confidential or personally identifiable information.
